Approved by CMS January 8, 2021 for an unusually long 10-year period through December 31, 2030, built around an aggregate spending-cap/shared-savings financing model (not a per-capita block grant). Numerous amendments since approval, including 2024 additions such as coverage of diapers for members under age 2.
