Every row carries the effective date we have on file for that state and billing code. States revise fee schedules on different cadences — some quarterly, some only when a specific rate changes — so always confirm against your state Medicaid agency before billing off any rate in the CSV.

What's the difference between Medicaid and Medicare reimbursement rates?

Medicare publishes national fee schedules with rate ceilings adjusted for geography and, for some categories, competitive bidding. Medicaid rates are set independently by each state — some peg a percentage of the Medicare rate, others publish an entirely separate fee schedule — and are frequently lower and revised on a different calendar than Medicare's.
