Why Medicaid rates are hard to find
Unlike Medicare, which publishes one national fee schedule, Medicaid is administered separately by all 50 states. Each state sets its own reimbursement rates, publishes them in its own format — PDF, Excel workbook, sometimes a legacy XML or XLS export — and revises them on its own calendar. Finding the current rate for a single billing code in a single state can mean digging through a state Medicaid agency's provider portal; finding rates across several states means repeating that search over and over.

Is there a single national Medicaid fee schedule?
No. Medicaid is administered separately by each state — there's no single national fee schedule like Medicare's. Every state sets its own reimbursement rates, publishes them in its own format (PDF, Excel, and in a few states, legacy XML or XLS exports), and updates them on its own schedule.
How current are the rates in this dataset?
Every row carries the effective date we have on file for that state and billing code. States revise fee schedules on different cadences — some quarterly, some only when a specific rate changes — so always confirm against your state Medicaid agency before billing off any rate in the CSV.
What's the difference between Medicaid and Medicare reimbursement rates?
Medicare publishes national fee schedules with rate ceilings adjusted for geography and, for some categories, competitive bidding. Medicaid rates are set independently by each state — some peg a percentage of the Medicare rate, others publish an entirely separate fee schedule — and are frequently lower and revised on a different calendar than Medicare's.

Our dataset spans billing codes across the Medicaid fee schedules we've collected and normalized state by state, including fee-schedule rates, Medicare-crosswalk rates, and manually-priced (MSRP) rates where a state doesn't publish a fee-schedule rate for a code.
