Why Medicaid rates are hard to find

Unlike Medicare, which publishes one national fee schedule, Medicaid is administered separately by all 50 states. Each state sets its own reimbursement rates, publishes them in its own format — PDF, Excel workbook, sometimes a legacy XML or XLS export — and revises them on its own calendar. Finding the current rate for a single billing code in a single state can mean digging through a state Medicaid agency's provider portal; finding rates across several states means repeating that search over and over.
